Per HA Policy 10-001, the health care provider should determine when a continuation of behavioral health care is warranted among a transitioning service member. If the service member agrees to transition coaching, the provider should work collaboratively with the service member to initiate the referral to the inTransition program. This referral must be documented in the service member's record.
