
Culture Survey
Thank you to all who completed the 2011 Tri-Service Survey on Patient Safety.
The Department of Defense (DoD) Tri-Service Survey on Patient Safety (Culture Survey), sponsored by TRICARE Management Activity (TMA), is an anonymous web-based survey designed to assess staff opinions about patient safety issues in the Military Health System (MHS). The most recent survey conducted in Spring 2008, invited all staff working in Army, Navy, and Air Force Military Treatment Facilities (MTFs) world-wide were invited to participate in the data gathering effort. The Tri-Service Survey on Patient Safety was fielded - once again - to all direct care facilities in the MHS in the Fall of 2011.
The purpose of the survey is to:
- Learn about staff opinions of patient safety culture in MHS facilities
- Raise awareness about patient safety issues
- Prioritize safety efforts to continue to provide safer care
- Assess safety culture changes over time

Results of the 2011 Tri-Service Survey on Patient Safety are forthcoming. The survey results will convey staff perceptions on areas essential to a safety culture, including: reporting errors, communicating feedback on errors, learning from errors, working in teams, handling care transitions and engendering management support for patient safety.
