| PSP Announcements:
Welcome to the PSP eBulletin We are here to eliminate preventable patient harm by engaging, educating and equipping patient-care teams to institutionalize evidence-based safe practices. We can’t do it without input from the experts – you. We are pleased to introduce the first DoD Patient Safety Program (PSP) eBulletin. We’ve heard from you, our stakeholders who make up patient-care teams. Patient Safety Managers, MTF leaders, Quality Managers and other MHS healthcare staff told us you want to receive patient safety information, but that you want it streamlined, monthly, and via email. Our new PSP eBulletin is our answer for you. With this email, you will receive information on PSP program updates, news, tips you can use, success stories and upcoming educational activities. Keep your eye out for more opportunities to opt-in to receiving tailored information. In the meantime, send us your feedback. Thank you for all that you already do to heal patients entrusted in our care. Lt. Col. Donald Robinson, Director
DoD Patient Safety Program Monthly Note:
Leadership Should Expect Zero In the August Commander’s Patient Safety Forum, we discussed: Expecting Zero: The Commander’s Role in Preventing Patient Harm. Brig. Gen. Wyman spoke about his experiences with TeamSTEPPS at Keesler AFB. While it’s against our nature to think that anything “null” can be a good thing, it’s imperative that we move the bar to zero preventable harm to patients. Commanders and MTF leadership need to expect teamwork and collaboration. They need to expect that their staff will feel empowered and equipped to speak up. And in return, staff needs to expect that they can communicate any concerns in a non-punitive, supportive environment.. If you are an MTF leader and would like to join our regular Commander’s Patient Safety Forum, email Lt. Col. Robinson at Donald.Robinson@tma.osd.mil. Share Your Patient Safety Story Do you have a safety success story? An emerging practice that you’ve seen work well? Please e-mail us to submit your story. We hope to share your stories with our readers and site visitors, so please note that we will not publish these stories without making sure identifying factors are removed. We look forward to hearing from you. Questions for the

But at many hospitals, the active ingredient to transform a simple checklist into an effective tool is often missing: an energized work culture that empowers everyone who takes care of patients to speak up. | | | | |
