Objective:
After attending this learning webinar, attendees will better understand the impact avoidable readmissions have on the cost and quality of health care throughout the Military Health System (MHS). National readmissions data will be presented and national strategies to reduce avoidable readmissions will be introduced. Attendees will be provided tools and best practices to mitigate the risk of avoidable hospital readmissions to patients in the MHS.  In addition, Patricia Rutherford, VP, IHI will be speaking about the STAAR collaborative.
