The Pharmacy Operations Division developed the WTU report in order to support the “Warriors in Transition High-Risk Medication Review and the Sole Provider Program.” >>View the WTU-PMART Presentation
The objective is to identify individuals who may require a more intensive medical review, to identify potential at-risk patients or to monitor adherence to a sole provider program. The data is delivered as a menu-driven, Microsoft database with individual look-up features and predefined reports designed to serve the needs of the health care provider(s) involved in the care of the WTU service members.
Services Offered
The following services are available to the Army, Navy, Marines, Air Force, and Coast Guard:
- Comprehensive prescription data of service members in WTU from all points of Service (military, home delivery, network)
- Analysis of data to identify high-risk individuals based on local Commander’s policy.
- Specialized medication reports focused on specific medication categories (i.e., Narcotic use, sleep aids, etc.)
We can provide the requested data within 5 business days or sooner if needed.
How to Get a WTU P-MART Analysis
- Complete the DHA Form 249: Pharmacy Data Warehouse Data Report Request Form
(To download form, right-click on the link and select "save link as" to save the form to your computer. To open, right-click on the form and select open with Adobe Acrobat.)
- Include Requestor's signature
- Send the form via email to the Data Management Team
Upon receipt, a member of the DMT will email you with instructions on sending your Unit’s Alpha Roster. The Alpha Roster should be compiled in an Excel Spreadsheet and needs to include the Service Member’s:
- Last Name, First Name & Middle Initial
- Rank/Rate
- Full Social Security Number
- Unit Name/Location
All Alpha Rosters will be submitted by the Requestor via Encrypted e-mail directly to the DMT Member who contacts you. If the Requestor is not contacted by a DMT Staff Member within 24 hours, please call 866-275-4732 or 210-536-6650 and leave a voicemail. A member of the DMT staff will contact you as soon as possible.
The completed WTU/P-Mart is delivered as a Microsoft Office Access 2000 Database. If you have an earlier version of Access, please let us know and we will create a version compatible with your computer.
Instructions on accessing the Secure Server for retrieval of your WTU/P-Mart will be e-mailed directly to the Requestor. Downloading instructions and necessary passwords will be sent under separate cover for security purposes.
