The Deployment Health Assessment Annual Summary 2021 provides a descriptive summary of the numbers of forms (pre-deployment, post-deployment, and post-deployment reassessment) submitted by month along with the percentages of service members who endorsed selected health status questions or received referrals on health assessment forms during calendar year 2021.
