Notice of Privacy Practices

The Military Health System (MHS) Notice of Privacy Practices (NoPP) is a notice that explains: 

  • How the MHS will use your protected health information (PHI);
  • When and where the MHS may disclose your PHI;
  • How to limit where the MHS will disclose your PHI; and
  • How the MHS will protect your PHI.

DHA published a new NoPP on October 1, 2013 making the 2003 NoPP void. You can get the new NoPP from any military hospital or clinic.

MHS NoPP Acknowledgement Form


The MHS Notice of Privacy Practices (NoPP) describes how medical information about you may be used and disclosed and how you can get access to this information. This form serves as an acknowledgement to patients and beneficiaries that they have received the MHS NoPP. The template is sized to scale and can be reproduced locally on Avery Label #5163, 5263, 2163, 5923, 5963 (size: 2 inches x 4 inches).
